  1. Cooperation in making and keeping appointments. Cancellation fee of $50/hour without 24-hours notice.
  2. A conscientious effort toward good oral hygiene.
  3. Recall visits to maintain optimum oral health.
  4. Payment of fees at the time of service. Cash, check, MasterCard or Visa is accepted. A finance company is available for your convenience.
  5. If using dental insurance, bring your dental benefits card and identification. If this information is not supplied, the full fee will be collected until your information can be received and verified.
